Case Managment is an integral part of the comprehensive health care of all patients admitted to the medical center. The nurse case manager works with physicians, managers, nurses, therapists, dietitians, pharmacists and any other hospital personnel involved in your care, as well as with other case managers and persons at your insurance company. The case manager's objective is to assist patients and their physicians in planning for discharge and accessing any needed supplemental care after discharge.
